Parent Survey

Dear Parents,

We have been talking with several students and parents in order to get a better understanding of how to better serve the students here at Calvary Chapel. The idea of a pre-teen youth group (5th and 6th graders) has come up. The goal is to give pre-teens the practical tools and Biblical foundation to successfully attack their junior high and teen years in Christ. Below is a questionnaire that will help us see how to best approach this ministry to your son/ daughter. Please fill out the questionnaire below. We appreciate your help and insights as we hope to have this ministry kick off in January of 2010. Thank you and God bless you.

Part #1 Student Information: This information will help us know what demographic of students we serve most here at Calvary.
1. Is your son or daughter currently involved in the AWANAS program at Calvary?
2. Which grade is your son/daughter currently attending: the 5th or the 6th grade?
3. Is your son/ daughter a part of the Boise School District or another local district?
4. How would you describe your son or daughter’s spiritual standing?


Part #2 Your Thoughts: This section is a portal to our understanding your desires and issues you may deal with at home.
5. If you would please share your insights on what would be important issues to discuss with your 5th or 6th grade student(s). (Based upon issues you may face with him/her at home). What subject(s) would you suggest we address?
6. We would love any ideas for outings (trips/ activities) you may be able to suggest for us to do with the 5th and 6th grade students. Please share any ideas you have or hobbies your child may already be involved in or expresses interest in.
Parental Involvement: We are always looking for parents to be involved with our youth ministries and would like to make you aware of a few opportunities for you to get involved. If you are interested in any of the areas below or may have ideas about how parents can get further involved then please fill out this section below.
7. Would you be willing to help with baked goods and snacks for an event?
8. Would you be willing to help chaperone for an event?
9. If needed would you be willing to help drive kids to/from an outing with your car/ a church vehicle?
10. Do you have a commercial driver’s license with a passenger endorsement that you would be willing to use in order to help us transport kids to and from outings?
11. Are there any needs that you may see or have regarding this age group as well as a possible solution to fill these needs? (please explain below)
